  1. The adjusted models presented in the final column include all variables in Table 1 and wave of survey. The pattern of adjusted results was unchanged in sensitivity analyses in which the adjustment for social grade was entered into the models as a 5-level categorical variable or age was entered as a 6-level categorical variable instead of the dichotomised and continuous versions of the variables reported in Table 1 respectively
